Where the document comes from — country and body
Where it is going — country and body
Whether the original is paper or digital
A digital copy of the document

For interpreting

Send an email or WhatsApp message, or telephone, telling me:

What the appointment is
Date and time
Where it takes place

Two things worth knowing

For a quote, the original document is not needed: a digital copy is enough. For a certified translation the original is required, because it is bound in with the certification.
The quality of the source document — paper, scanned PDF, Word file — affects the formatting work involved and therefore the quote. A legible scan saves you money.
